The goods and services listed below are what we can provide to our customers. You may choose only the items needed. Any services
you choose will include a charge for our basic services and overhead. If legal or other requirements mandate you to purchase any items you did not ask for, we will explain the reason in writing on the goods and services statement.

Mileage outside of service area: $3.25 per mile
Certified copy of State of Ohio Death Certificate: $35.00 each
Affidavit filing fee of Ohio Certified Death Certificate: $35.00 each
Certified Copy of Kentucky Death Certificate: $15.00 each
Domestic shipping through United States Postal Service: $129.00
Transfer of cremated remains to urns purchased from vendors other than Cremations Direct: $25.00
Obituary Submission Fee*: $75.00
* Does not include the charges for placing obituary in local papers
Memorial Package (Register book, 100 memorial folders or prayer cards, 50 acknowledgement cards): $175.00
Additional fee for decedent over 350 lbs to cover additional crematory and removal costs: $250.00
Insurance Processing Fee: $250.00
Urns: From $50.00 - $500.00
